Breakthrough Chemo-Free Regimen Slashes Mantle Cell Lymphoma Risk by 43%

BeOne Medicines Ltd. has reported positive topline results from its Phase 3 MANGROVE study. The trial compared a chemotherapy-free regimen of BRUKINSA plus rituximab against standard chemoimmunotherapy in previously untreated mantle cell lymphoma patients. Mantle cell lymphoma is a rare and aggressive form of B-cell non-Hodgkin lymphoma. It most commonly affects older adults and requires effective yet tolerable treatments.

The MANGROVE trial is the first Phase 3 study to test a BTK inhibitor-based chemotherapy-free approach against the standard bendamustine plus rituximab (BR) regimen. Chemoimmunotherapy like BR has long been the frontline treatment but often causes myelosuppression, prolonged immune suppression, and cumulative toxicity.

The trial met its primary endpoint, showing a 43% reduction in the risk of disease progression or death for patients on BRUKINSA plus rituximab (HR=0.57). Overall survival data also favoured the new combination, though the results remain immature at this interim analysis. The safety profile of the regimen aligned with the known profiles of both drugs. Full findings from MANGROVE will be presented at an upcoming medical meeting. Regulatory submissions for the BRUKINSA plus rituximab regimen are planned for the second half of 2026. The results suggest a potential shift away from upfront chemotherapy for some MCL patients.
